As with any training programs, training facilities, leagues, 1-on-1 sessions, basketball events, or any other services of that matter, participants must acknowledge and understand that there are risks, including accidental and bodily injuries.
By accepting this waiver, you agree to assume all responsibility and liability for all accidents, and waive any claim for personal risk, bodily injuries, ankle sprains, ACL / MCL / PCL tears, emotional damages, or psychological damages. You confirm that, to your knowledge, the athlete has no limiting physical conditions or disabilities that would preclude any services or events rendered by CROSSED ATHLETICS or KIDFIT SPORTS ACADEMY LLC.
In case of emergency, the emergency contact provided above will be notified immediately. Crossed Athletics trainers will act accordingly, which may include: contacting your emergency contact, calling for emergency or medical assistance if needed, and moving the injured participant aside for further care.
By accepting below, you take full responsibility for the athlete's health and well-being and acknowledge that no responsibility is assumed by CROSSED ATHLETICS or KIDFIT SPORTS ACADEMY LLC, its authorized managers, trainers, referees, or program leaders — including for COVID-19-related illness and, in extreme cases, death.
No alcohol, drugs, marijuana, or any illegal substance may be brought to or used around our premises (Lakeview Plaza, 11407 NW 122nd St., Medley, FL 33178). Violation will result in immediate contact with law enforcement. Crossed Athletics does not stand for that kind of behavior. Thank you & God bless!
